Complete each tier to be entered into the prize draw. E.g. to be entered into the Tier 3 prize draw, you must also attend eligible activities weekly and complete Tier 1 and Tier 2. After each visit and participation in an eligible activity, participants can record 1 ‘tick’ on their activity card. Maximum of 1 tick per visit.

• Speak to a member of the leisure centre reception team for a full list of the activities included in this prize draw.
• Submit your completed card to reception by 30 April 2022
• If you fill in a card, collect another for more chances of winning
• Centres included in Better You in 2022: Clifton Leisure Centre, Djanogly Community Leisure Centre, Ken Martin Leisure Centre, Southglade Leisure Centre, Harvey Hadden Sports Village and Victoria Leisure Centre
• Must be a Fitness+ (formally Health & Fitness memberships) or Swim+ (formally Swim & Health memberships) member to take part
• Gain an extra bonus entry by completing the four additional bonus challenges
• Selected winners agreed to have their image shared on Active Nottingham’s social media channels and marketing material.
